Carefully review the instructions below to ensure your request for consideration in joining California Mental Health is complete.

Apply Manually:

1. Download and save the Profile Intake Document to your computer.

 

2. Complete the Practice Profile Form.

3. Email to: info@californiamentalhealth.net

Apply Electronically:

 

 

 Start with Part 1 and work through to Part 3. Each Part is 

 submitted separately, so feel free to take a break between

 each section. You will receive a notification as you

 progress. After all three are submitted, a representative

 will contact you within 1-3 business days.
